Our care managers are a multi disciplinary team of Social Workers, Registered Nurses, Certified Diabetes Educators, and Clinical Pharmacists that help manage conditions and improve clinical measures. This results in lowering costs for those with diabetes, coronary artery disease, hyperlipidemia, hypertension, and more!

Based on our experience, the earlier we intervene with individuals the better. Therefore we proactively identify and intervene with members with early indications of chronic conditions to prevent those conditions from developing.

Combining our clinical expertise with health care data, knowledge of employer-specific benefits, and our coordination with physicians, our Clinical Pharmacists are able to affect significant savings on prescription drug costs for both employers and their employees.
