Who should you refer to this program?
- Adult patients with Type 1 or 2 diabetes (gestational diabetes is excluded)
- Patient has an A1c > 8% or;
- Patient is on insulin or;
- Patient is on > 2 oral medications for diabetes
- Newly diagnosed
A wide range of interventions and approaches
Provider/Staff Education
- Small group educational sessions
- Staff survey to identify key issues of concern
- Clinical guidance in implementing standards of care
- Staff training to perform/document preventive care measures
Group Visits
- 4 – 8 patients per group
- 2-hour sessions for 4 consecutive weeks
- Empowerment approach to establish individual goals based on AADE 7 Self-Care behaviors
- Overall assessment and monitoring of Diabetes Standards of Care
Individual Visits
- 1:1 consultation on a limited basis for patients unable to attend group visits or as requested by the primary care provider at point of service
