Tom Sorrento
(Rochester, NY, April 11, 2007) Tom Sorrento, RPh, a Consulting Clinical Pharmacist for the Greater Rochester Independent Practice Association, is the recipient of the 2007 Max M. Farash Award for Excellence in Aging and Long Term Care Services from Lifespan.
Sorrento, a resident of Webster, specializes in geriatrics and works along with physicians, nurses and social workers as part of GRIPA’s Geriatric Assessment team. Sorrento also provides medication management services to Independent Living for Seniors, helped launch “Healthy Aging” classes and is part of the GRIPA Diabetes team which provides support and education to patients.
The award is given in honor of Max Farash whose support of Lifespan in the early 1990s (then the Regional Council on Aging) was unwavering.
The Greater Rochester Independent Practice Association is a partnership of physicians and hospitals in Monroe and Wayne Counties which includes Rochester General Hospital and Rochester General Physicians Organization, Newark Wayne Community Hospital and the Wayne County Physicians Organization. GRIPA has been in the Rochester Top 100 for the past three years.
